Home and Community Based Services (H&CBS)This program is designed to keep families intact. Staff is assigned one on one relationship to
work with the person who has developmental disabilities. The areas of service would include a
specialized plan of action which guardians have agreed to support. Each person has five specific attainable
goals to be addressed. A team approach
with professionals, parents, and guardians is utilized. The team may consist of a physician, nurse,
consultants such as a social worker, recreational therapist, dietitian,
psychologist and nurse with parents and guardians who contribute to this
overall effort.
This program offers the person who is disabled an
opportunity to have a Personal
Assistant to enhance their lifestyle.
This person may be a friend, family member, or someone from the
community. They will provide the needed
supports requested by the person who is disabled. Some examples of support may include assistance with community
outings, learning skills, craft or art projects, transportation, personal care
or supervision. When a Personal Assistant joins our trained
staff, they must meet certain employment requirements.
